Welcome to the Pacific Northwest Headache Clinic.  Dr. James A. Moren, is committed to serving headache sufferers with the highest-level care and treatments possible. He currently is accepting new patients.

PLANNING YOUR VISIT

First time patients should plan to arrive 45 – 60 minutes early to complete patient registration forms.

As a new patient, you can expect your consultation to be between 60 – 90 minutes.  In preparation for this appointment, we request that you bring in the following forms completed and signed:

Open the below form, but you don’t need to print it. This is a customary HIPPA privacy practice form, so print if you like, but please skim it enough to confirm it is like other standard privacy practices. No need to sign it, but on the new patient registration form you will need to sign that you are aware of our privacy practices.

Please also bring any recent imaging (MRI or CT scan of head and neck or lab results) you have to the appointment, and a list of all prescriptions and over-the-counter medications.  This will allow for a more productive and efficient consult between you and your doctor.

A receptionist will answer all clinic calls Monday through Friday from 8 am to 5 pm.  If Dr. Moren is available, all calls will be immediately forwarded to him.  If he is not available, please leave a voice message and Dr. Moren will return your call.

The Pacific Northwest Headache Clinic is located on the 4th floor of the Crown Plaza building, Suite 439.

Free parking is available for patients on the Northeast side of the building.  There are 7 designated spaces available, and parking is limited to 2 hours.  Otherwise, street parking and a nearby parkade are alternative parking options.

INSURANCE, FEES & PAYMENT INFORMATION

Pacific Northwest Headache Clinic does not accept insurance.  We are a cash only clinic – cash, checks or credit cards (Mastercard, Visa or Discover).  The patient will be provided with a receipt which they can send to their insurance companies for possible reimbursement.  The initial appointment fee for one hour is $300.  Follow up appointment fees are variable, and time based.

CANCELLATION POLICY

We realize that our patients’ time is valuable, and we make every effort to see patients at their scheduled time.  We ask that patients provide as much advanced notice as possible when running late for an appointment.  If you want to cancel, please give us at least 48 hours’ notice.  With this advanced notice, the time can be scheduled for another patient.
